Partner with a health coach nurse to map out your wellness plan! Together, you will build a bridge from where you are now to where you'd like to be.

Use a whole-being assessment to clarify your current wellness state.


Determine where you'd like to be

What does your ideal level of wellness look like? What would you like to be able to do that you can't do now?


Find your motivation for change

How important is making a change to you right now? How confident are you that you CAN DO THIS?


Discover your strengths

Building on your strengths is key to whole being wellness.


Set a goal

Set a goal that is specific, measurable, attainable, relevant, time-based, inspiring and excitiing! 


Work towards the goal

Develop self awareness, learn to manage challenges and celebrate successes.


Re-evaluate

At the end of your time, evaluate your outcomes together, including successes and areas of continued effort.


  • Six Month Health Coaching Membership

    Set a goal and get to work! Six Months is a great amount of time to see results. 


    A six month membership includes six in person sessions and a weekly vitural check in via email. In person sessions can happen at the Decatur Wellness Collective, or off site at a location that works for you!

  • Provider Appointment Preparation

    Sixty to ninety minute session. Be well-prepared for that important appointment you have coming up!  Provider appointments are often kept to a time limit.  This can make communication between your provider and you feel rushed and sometimes result in unanswered questions.  Meet with our health coach nurse to create a plan for getting the most out of that appointment.

  • Understanding Your Lab Work

     Sixty to ninety minute session. Bring your lab results to review with our health coach nurse.  Gain a basic understanding of the lab tests and receive guidance on questions to ask of your provider related to the results.

  • Free Pop Up Walks!

    Libby hosts regular pop up walks in the community. This is a great way to stay active and socialize. Pop up walks happen year round, and we have a text list so you will be personally notified when one is happening!

Libby Pasquariello

Libby Pasquariello, MSN/ED, RN, HWNC-BC, is a health coach, educator, and patient advocate.  A registered nurse for twenty years now, she spent the first ten of those working with adult clients in mental health care settings.  During this time, she experienced the beauty of nursing through the interpersonal connections made with clients and families, but she also recognized the limits of the health care system.  She returned to complete graduate education as a nurse educator and has spent the last decade teaching in higher education.  After earning certification as a health coach nurse, now she aims to connect with individuals and small groups seeking to improve their overall wellness and quality of life.  She believes that illness and wellness lie on a spectrum and absence of illness does not always mean wellness.  There is a gap between the care provided by the traditional health care system and people’s daily lifestyles and this is where health coaching with a nurse can provide a bridge. 


 The Quiet Cornerstones of Well-Being:  Libby believes stress relief and the power of rest are undervalued in today’s society, resulting in a culture that celebrates busyness over quality of life.  She believes finding joy and cultivating a sense of playful curiosity about life are tangible skills for anyone seeking to improve their overall wellbeing and quality of life.  And she believes YOU should be the one to define what wellness looks like for you.
